a 55 year old lady presented to emergency department with severe earache and sore throat. she was agitated showing her throat with both hands. she described sore throat as choking sensation and earache as intense fullness. she reported that she had the feeling of stop of breathing

she presented to emergency department about 6 hours ago with mild pain while swallowing and earache since 2 days. She appears restless in her first visit. Her blood pressure slightly high 135/70, heart rate 70, respiratory rate increased it's 22 per minute[normal 12-16]. heart, lung, abdomen physical examinations all normal

because her father died of heart attack , she had ECG done and cardiac enzymes all are normal. she was treated symptomatically and discharged

on her second visit again to emergency her complaints were more severe .Her skin was pale and cold and far more agitated

this is a atypical case not the usual classic presentation but  intentionally did this to create awareness as all cases will not present with classic symptoms
