What about site visits for remote employees if they work from home? And if remote employees work from India for a few weeks – what’s a general rule how long they are allowed to work there?

If you work remotely from home on an H1B visa, your H1B LCA should explicitly mention your home address as one of the ‘work locations’.

H1B can legally work from home by filing an amendment.

To work from outside the USA, you have to understand the concept of status in the USA. I have explained how can H1B work from India or Canada (on PR) and what things to keep in mind.

for example I am working for a retail client 9-5; why can't I work for some other company say health care or financial client as long as second client know that my primary job is something else and I don't want to be bothered during 9-5. CC @Tellugodu and @Konebhar6

No conflict of interest here. But the new client allow you to work beyond 9-5? If your primary employer says 9-5 are your timings, you cannot allocate that time to others.

But in general, no one cares, no one knows. You are free to do whatever you want to with respect to 2nd job.

The majority of people, myself included, take on second jobs without disclosing them to either employer. As long as you fulfill your work responsibilities on time, it’s unlikely anyone will be concerned. These companies often don't hesitate to let employees go when it suits them, so there’s no need to seek their approval for what’s best for you. Prioritize your interests and avoid being overly loyal; companies can replace you at any moment.
