I will check for other secondary causes such as pheochromocytoma and conn's disease cushing's , and also rule out sleep apnea a very important cause and medication history such as NSAIDS

If none of them is present then it is primary hypertension then the stage of hypertension is important that is according to JNA 8 Classification that is it 120-140 or 140-160 etc

As you have mentioned no renal complications and also check for coronary artery disease and also neurologic complications

If none of the complications are present then it is uncomplicated hypertension

If it is stage 1 then suggest lifestyle modifications

If not controlled or in higher stage I start with calcium channel blockers in this patient considering his age like amlodipine 5-10 mg daily

While the prevalence of higher aldosterone and cortisol are good markers, as secondary causes, most of the times cases of uncomplicated hypertension are let go of by Indian doctors by writing pills without looking at insulin levels. Fasting blood glucose is often checked and then they stop there but many don't go to extent of checking higher insulin levels and metabolic disorder. India being a high refined carbs eating country it's prevalence is underreported. The higher insulin levels trigger both hypertension and atherosclerosis.

This is what I did with my own uncle,  when the doctor put him on Amlodipine, which gave him swollen knees and affected his libido so he chose to stay away from it. He switched to keto by cutting down rice portions drastically and his numbers are pre hypertension range within 10 days and continue to be so. He is pill free now. There is a reason why Veeramachineni diet has gained traction in Telugu states.

Dude we may have many differences but you do understand what am I trying to say in this thread  that low threshold of fat percentage depends on his HDL and LDL levels which determines the fatty acids transport to the tissues for metabolism

What do you mean ? If you're referring to the Y-Y Paradox, the right side guy has high BF %, whereas the guy to the left has low BF% (22 VS 9),he doesn't appear 9% that's a different thing though.

Where are you getting with this? Having low BF doesn't mean your LDL levels shall stay low, and having high BF% doesn't automatically mean having high LDL. The paradox if any reinforces the limitations of BMI.

I agree cutting the carbs helps but just not diet but regular exercise is important for controlling hypertension, and veeramachineni diet is trash and not scientific, it's quackery

If the hypertension I'm stage 2 you definitely need medication just dietary modification and exercise won't be sufficient

The other guy is the one arguing low body fat percentage does not depend on LDL and HDL, I am saying even if it is low body fat percentage it still depends on LDL and HDL levels

My uncle made this small change and he's off meds now.Ketogenic diets act as natural diuretic. Veeramachineni diet is indianized or rather Andhra version keto and carnivore depending on your ingredients.
