Collect below information , last points (e) and (f) is optional 
a) L1A Full Name of L1 Employee Name 
WITCH Employer Name, Which Client 
c) Client Address/worksite 
d) A brief write up of L1 beneficiary job profile, asset beneficiary working on Client Project in body shopping 
e) Client Manager/Technical/QA Contact Name & Phone Number whom this L1 beneficiary is reporting. 
f) Provide screen shots of emails/proof of the L1 employee working for client projects directly.
