He then also asked for proof that my green card I-140 had been approved. I did not have the original I-140 approval. I did show him the relevant approval which was attached to my H1B extension petition. He was not initially convinced but checked with a colleague who said its ok. He then came back and said “sorry for the delay, your visa is approved”. 

It was clearly my mistake in not taking the I-140. Otherwise the experience would have been extremely smooth. As others have said best to just take every possible document with you.

not true.. if you use 140 for H1 extension beyond 6 years..they may ask to show 140 approval..
