Index funds pay dividends..not as much as dividend portfolio's though...you have to file tax returns for dividends unless you are buying them in IRA or 401K.

Both are same...ETF is when you would like to invest smaller than the fund minimum amount..ETF lo esko if it's not  401k...

90% SPX index fund (SPX) and then 10% bonds should provide you what your looking for
