Apply online you will get in a week from day they receive the finger print cards. Google fingerprinting locations in your city. Usually from police station or city citizen services office.it cost me about 3$ per copy. Depends on location

a person who has been living in US for past 15 yrs should also give an IELTS test to prove his english efficiency???

If your experience did not give that confidence then that is a different issue. Soĺve it first.
