Yes. You will mostly need affidavit unless officer is in relax mode and ignores. if you already filed then just hope they ignore. If you are in process of filing in future then get affidavits too. 

As long as birth city name matches you should be fine.  Passport lo just city name vuntadi kada, how did you got area name too ? Home address em vunna parledu. Bio page lo city/place of birth vuntadi, adi important.

After looking disco on birth certificate I took a peek of my birth certificate and found mistakes. I got bc from municipality they mentioned mother name spelling incorrect. ***ba badulu ***bha ani undi. Also date of registration is application date mention chesadu. 
 

What to do guys? Mi mother is no more for affidavit. How can the date be corrected?
