fill new DS160... update profile and then print confirmation...it will reflect new info

yes...u can also try sending an email to ustraveldocs... they can also update. if not, direct dropbox appudu they will be able to change it

i think they accept. i once gave country as US for indian address  

they asked to create new one and bring. they will scan barcode  and attach it to the case

yeah. vaccination card or certificate unte saripothundi for coming from USA. but airline of transiting country might ask for one.
